Responsibilities
- Conduct comprehensive occupational therapy assessments for residents in a skilled nursing setting.

- Document progress, adjust treatment plans as needed, and communicate findings to the interdisciplinary team.
- Educate residents and families on therapy goals, home exercise programs, and safety strategies.
- Collaborate with nursing, therapy, and social work staff to support discharge planning and transitions of care.
Requirements
- Active OT license in Vermont or eligibility for Vermont licensure
- Degree in Occupational Therapy from an accredited program (Master’s or Doctorate)
- Ability to work effectively in a skilled nursing facility setting
- Strong assessment, documentation, and treatment-planning skills
- Excellent communication and teamwork abilities
