Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual, and cultural factors and incorporate that data into the POC.
- Maintain up-to-date patient records so that visits, contacts, problems, plans, actions, and goals are accurately stated and updated as changes occur.
- Provide care using infection control measures; educate patients and families on care, medications, home safety, and related topics; promote self-care practices.
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality and continuity of service.
Requirements
- Current Arizona RN license; active state licensure required
- ADN or Nursing Diploma program graduate; BSN preferred
- 1–2 years of RN experience in a clinical care setting; 2+ years of home health experience preferred
- OASIS experience required; Homecare Homebase software experience required
- CHPN certification preferred; CPR certification required
- Valid driver’s license and reliable transportation with insurance; ability to travel locally
- Ability to assess and care for patients in home settings; strong communication and documentation skills
