- Transportation note: Public transit is not reliable in the area; reliable personal or rental transportation strongly recommended

Responsibilities
- Deliver direct, supervised nursing care to LTC residents in accordance with the plan of care and licensed scope of practice
- Administer medications and treatments per facility policy; perform basic nursing procedures as needed
- Monitor resident status, vital signs, and response to treatments; identify and report changes promptly to the supervising RN
- Assist with activities of daily living and related CNA duties as needed to support resident comfort and safety
- Document care provided and resident responses in the charting system per policy; maintain accurate, timely records
- Collaborate with the interdisciplinary team to support individualized resident goals and quality care

Requirements
- Active New York LPN license; or proof of application with applied date (NY licensing may take 6–8 weeks; start dates/offers will consider this timeline)
- BLS (AHA) certification required
- Minimum 1 year of LPN experience, with LTC/long-term care experience preferred
- Ability to perform LTC nursing duties in the evening shift and work every other weekend; participate in holiday coverage as required
- LPN may be asked to cover CNA duties as needed
- Reliable transportation is strongly recommended due to limited public transit in the area
