Responsibilities
- Conduct comprehensive physical therapy evaluations and develop plans of care for residents
- Implement therapy interventions (gait training, mobility, balance, ROM, strength) and utilize appropriate modalities
- Monitor patient progress, adjust treatment plans, and document progress notes in a timely manner
- Collaborate with nursing, occupational therapy, speech-language pathology, and other members of the rehab team; participate in care conferences
- Ensure safety, infection control, patient education, and discharge planning; maintain privacy and HIPAA compliance
Requirements
- Active Physical Therapist license in Pennsylvania or eligibility for licensure
- Ability to work effectively in a skilled nursing facility setting
- Prior clinical experience in physical therapy; SNF or rehab experience preferred
- Willingness to travel for a temporary assignment and adhere to the 14-week duration
- Strong documentation and communication skills
