Responsibilities
- Administer prescribed medications and monitor for therapeutic effects and adverse reactions in collaboration with the supervising clinician.
- Perform comprehensive nursing assessments, document findings, and contribute to individualized resident care plans.
- Monitor residents for changes in condition, respond to acute needs, and escalate concerns promptly to the care team.
- Provide direct patient care duties such as vital signs, wound care, and routine procedures per scope of practice and facility policy.
- Maintain accurate and timely documentation in the electronic health record, ensuring compliance with regulations and privacy standards.
- Collaborate with registered nurses, physicians, and other disciplines to coordinate resident care and safe discharge planning when appropriate.
Requirements
- Active LPN license in good standing with eligibility for practice in the United States; Vermont licensure or endorsement eligibility may apply.
- Minimum 1 year of clinical LPN experience; long-term care experience preferred.
- Ability to work evening and night shifts; flexible schedule to meet resident needs.
- Strong documentation, assessment, and communication skills; comfortable working in a team-based setting.
- Physical ability to perform essential nursing tasks in a long-term care environment, including lifts and transfers as required by policy.
- Willingness to travel for assignment and comply with facility policies and onboarding requirements.
