Responsibilities
- Assist with and supervise the nursing activities of the facility to maintain the highest level of quality care at all times.
- Administer medications, monitor resident status, and document changes for the care team.
- Collaborate with an interdisciplinary team to implement individualized care plans, including dementia/memory care considerations.
- Monitor safety, infection control, and regulatory compliance; complete required documentation and reports.
- Provide direct patient care, including assessments, vital signs, and basic wound and symptom management as appropriate.
- Communicate with residents, families, and team members to support ongoing care needs and transitions.
Requirements
- Colorado Registered Nurse license (current/active) and ability to practice in the state.
- Colorado adult protective services (CAPS) check required; willingness to undergo the CAPS screening process.
- Ability to work day shift on a 3x12-hour schedule (6:00 a.m. – 6:00 p.m.) in a memory care-focused LTC setting.
- Willingness to relocate to Gunnison, CO for a 13-week travel assignment with reliable transportation to and from work.
- Strong communication and teamwork skills with a focus on resident safety and quality of care.
