Travel Registered Nurse Case Manager Job
Registered Nurse – Case Manager (Travel) in Everett, WA, starting ASAP for a travel assignment spanning Weeks in an acute care setting. The role centers on acute care discharge planning and care coordination with 72 guaranteed hours per 2-week period, on a 5x8 daytime shift. Work in a Level II trauma center with diverse patient populations and an emphasis on next-level care transitions, charted in EPIC. The assignment involves floating to related units and between two nearby campuses to support the discharge process and care coordination.

- Orientation and onboarding: ~30 hours of orientation; pre-start modules typically required (average ~16 hours)
Responsibilities
- Perform discharge planning and care coordination for acute care patients across a broad age range, from neonates to geriatrics
- Assess patient needs, develop and implement an appropriate plan of care, and coordinate next-level care (e.g., home health, rehab, facility care) including DME orders
- Collaborate with Social Services and the interdisciplinary team to ensure safe transitions of care and appropriate placements
- Maintain thorough documentation in EPIC, including needs assessments, plans of care, and progress notes; monitor caseload of approximately 25–30 patients
- Educate patients and families on post-acute services, medications, and follow-up needs; coordinate with payers and post-acute providers as required
- Float to related units and between campuses as needed to support discharge planning and case management coverage
Requirements
- RN licensure (pending accepted) and BLS certification
- 1 year of acute care experience with discharge planning / care coordination required
- Strong experience in discharge planning / care coordination; diverse case management across Med-Surg, LLOS, and ED preferred
- EPIC charting proficiency; ability to document comprehensively and navigate the electronic health record efficiently
- Ability to work in an acute hospital setting and to float between units and campuses; effective collaboration with Social Services and interdisciplinary teams
- Must commit to working 3 of the 5 major holidays; background checks and references required (minimum 2 supervisory references within the last 3 years); pre-start modules (~16 hours) and orientation (~30 hours)
