Responsibilities
- Provide direct patient care to residents in the long-term care setting, including medication administration under supervision and routine nursing duties.
- Monitor resident status, perform routine assessments, document findings, and report changes to the supervising RN.
- Administer prescribed medications and treatments per plan of care; ensure accurate documentation and compliance with policies.
- Assist with activities of daily living, comfort measures, and safety needs; manage wound care and basic care planning as directed.
- Collaborate with RNs, physicians, and CNAs to implement individualized care plans; communicate resident needs and changes effectively.
- Maintain infection prevention practices, safety standards, and high-quality charting in the resident’s medical record.
Requirements
- Active LPN license in New Mexico in good standing (or eligible for licensure in NM).
- Ability to work night shifts (3x12) in a long-term care setting.
- Strong communication and documentation skills; ability to collaborate with an interdisciplinary team.
- Demonstrated ability to perform essential LTC nursing duties and follow established care plans.
- Eligibility to participate in a travel assignment and complete onboarding/credentialing requirements.
