Responsibilities
- Observe each resident continuously for changes in mental and physical well-being; notify the Supervisor as needed and provide a detailed handoff report at shift change.
- Accurately receive, transcribe, and carry out physician orders; administer medications and treatments and document in MARs/TARs and electronic systems.
- Order and maintain pharmaceuticals for units; supervise LNAs in accordance with NH Board of Nursing; assign and monitor nursing care within scope of practice; assist with direct-care tasks as time allows.
- Accompany the physician on rounds on the assigned unit(s) to address medical needs and support 60-day reviews with clear communication.
- Administer oxygen and gastrostomy/jejunostomy feedings per policy; perform wound care and manage wound vacs as appropriate.
- Complete unit duties such as coordinating appointments, therapy sessions, activity board updates, ambulation lists, vital signs, and nutritional charts; monitor LNA documentation and infection control practices; prepare 24-hour unit reports; assist with admissions, discharges, and transfers; communicate with families as needed.
Requirements
- Active New Hampshire Licensed Practical Nurse license
- BLS (AHA) certification; IV certification required or willingness to obtain during onboarding
- 3+ years of current LPN experience within the last 6 months
- Willingness to float to units where experienced
- Covid-19 vaccination required (declinations/exemptions not accepted)
- Non-local candidates only; must reside more than 50 miles from the facility and have own reliable transportation; previous travel experience and rural/small facility experience are preferred
