Responsibilities
- Conduct comprehensive PT evaluations and develop individualized plans of care for residents in a long-term care setting.
- Implement therapeutic interventions to improve mobility, gait, balance, strength, and functional independence in activities of daily living.
- Reassess residents regularly, document progress, and communicate changes in goals and plans to the care team.
- Collaborate with nurses, physicians, and other rehabilitation professionals to coordinate discharge planning and optimize resident outcomes.
- Ensure documentation adheres to professional standards, regulatory requirements, and facility policies; maintain patient confidentiality and safety throughout all treatments.

Requirements
- Active physical therapy license in good standing (state requirements applicable).
- Doctor of Physical Therapy (DPT) or eligible to practice as a PT; appropriate licensure for the assignment.
- Willingness to work in a long-term care environment and to participate in a travel assignment lasting several weeks.
- Strong communication skills and ability to work collaboratively with an interprofessional team.
