Responsibilities
- Assess residents’ physical therapy needs and develop individualized treatment plans focused on mobility, strength, balance, endurance, and daily living activities
- Implement therapeutic interventions to improve functional independence and reduce fall risk
- Document evaluations, goals, progress notes, and outcomes in the resident’s medical record with accuracy and timeliness
- Reassess patients regularly; modify treatment plans based on progress and current clinical status
- Communicate effectively with an interdisciplinary team (nursing, OT, SLP, and case management) to coordinate care and discharge planning
- Ensure patient safety, infection control, and privacy during all therapy services
Requirements
- Active Physical Therapist license in Washington or eligible for licensure in WA
- Demonstrated ability to work in a skilled nursing/geriatric rehabilitation setting
- Willingness to commit to a 13-week travel assignment with 36 guaranteed hours per week
- Ability to complete credentialing requirements and pass standard background checks for travel assignments
- Strong documentation, communication, and teamwork skills
- Ability to work the listed shift and adapt to the assignment schedule
