Responsibilities
- Manage the development, implementation and evaluation of the care management service lines for health plan members.
- Promote continuity of care and cost effectiveness through whole-person in-patient and out-patient management, care integration, case management, utilization review and care planning.
- Coordinate with interdisciplinary teams to support member outcomes, transitions of care, and appropriate resource use.
- Document care plans and maintain accurate records to inform care decisions and quality improvement initiatives.
Requirements
- Active RN license and current licensure as required by state regulations
- Ability to operate in a care management and care coordination environment
- Strong communication and collaboration skills with interdisciplinary teams
- Competence in care planning, utilization review, and documentation
- Availability to work 8-hour shifts, five days per week
