Responsibilities
- Perform initial evaluations and ongoing assessments of residents’ physical therapy needs
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
- Provide therapeutic exercises, gait training, range of motion, and pain management as clinically indicated
- Monitor progress, update care plans, document in the electronic health record, and communicate with the health care team
- Educate residents and families on home exercise programs and safety, and participate in discharge planning
Requirements
- Active California physical therapy license or eligibility
- Doctor of Physical Therapy (DPT) or equivalent degree
- Minimum 1 year of PT experience in a skilled nursing or related rehab setting preferred
- Ability to work in a travel assignment and in a skilled nursing facility environment
- Strong clinical documentation and communication skills
