Responsibilities
- Administer prescribed medications and treatments per the care plan and physician orders
- Monitor residents’ conditions, vital signs, and report changes promptly to the supervising RN
- Provide routine nursing care and assist residents with activities of daily living
- Document assessments, interventions, and outcomes accurately in the resident chart
- Assist with wound care, routine procedures, and specimen collection as directed
- Collaborate with the interdisciplinary team to ensure safe, person-centered care and regulatory compliance
Requirements
- Active Licensed Practical Nurse license in the state of New Hampshire
- Experience in long-term care or skilled nursing settings preferred
- Ability to work the listed schedule (7:00–15:30) and perform on-site in Bedford, NH
- Strong communication, documentation, and teamwork skills
- CPR/BLS certification preferred or required by assignment terms
