Responsibilities
- Provide direct patient care to residents within the scope of the LPN under the supervision of an RN
- Administer prescribed medications and monitor effects, reporting any concerns
- Conduct routine assessments, document vital signs, and communicate changes promptly
- Assist residents with activities of daily living and support care planning
- Monitor safety, assist with transfers and mobility, and support fall-prevention efforts
- Collaborate with RNs, physicians, and the interdisciplinary team to update and implement care plans
Requirements
- Active LPN license in good standing (state as applicable)
- Ability to work a 3x12-night shift schedule and maintain 36 hours per week
- Experience in a long-term care or skilled nursing setting preferred
- Strong documentation and communication skills
- Ability to follow facility policies and comply with regulations
- Eligible to work in the United States and meet assignment requirements
