Responsibilities
- Provide direct patient care to residents in a long-term care setting within the scope of an LPN practice
- Administer prescribed medications and document doses accurately
- Monitor vital signs, assess changes in patient status, and escalate concerns to the supervising RN
- Assist residents with activities of daily living and mobility to support independence and comfort
- Maintain thorough, compliant documentation and participate in care planning with the nursing team
Requirements
- Active LPN license (state unspecified)
- Current CPR/BLS certification
- Ability to work the evening shift (15:00–23:30)
- Experience in long-term care or skilled nursing preferred
- Education: graduate of an accredited practical nursing program
- Local candidates in the Keene, NH area preferred
