Responsibilities
- Conduct comprehensive physical therapy evaluations for residents in the skilled nursing setting and establish individualized treatment plans.
- Implement evidence-based therapies to improve mobility, strength, balance, and functional independence.
- Reassess resident progress regularly; adjust treatment plans and goals accordingly.
- Document care in accordance with regulatory requirements and facility policies; communicate progress to the interdisciplinary team.
- Collaborate with nursing, therapy, and other care colleagues; participate in discharge planning and care transitions.
Requirements
- Active Maryland Physical Therapy license.
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Experience in skilled nursing facility or geriatric rehabilitation settings preferred.
- Strong communication and documentation skills; ability to work within a multidisciplinary team.
- Ability to travel and adapt to a temporary assignment environment.
