Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for residents.
- Implement rehabilitation interventions targeting mobility, gait, strength, balance, pain management, and activities of daily living.
- Monitor progress, adjust treatment plans, and document therapies and outcomes in patient records.
- Collaborate with nursing, therapy aides, and case management to coordinate goals and discharge planning.
- Educate patients and families on home exercise programs, safety, and activity strategies.
- Ensure patient safety, equipment use, and adherence to infection prevention protocols during therapy sessions.
Requirements
- Active Physical Therapist license in Massachusetts or eligibility to obtain MA licensure.
- Doctor of Physical Therapy (DPT) degree or equivalent.
- Experience in skilled nursing facility or geriatric rehabilitation preferred.
- Ability to provide care across daytime hours with 36+ hours per week; willingness to travel for assignment.
- Strong communication and collaboration skills with an interdisciplinary team.
- Eligibility to work in the United States.
