Responsibilities
- Develop and implement a tailored plan of care for Home Health and Hospice patients, delivering skilled nursing services in patients’ homes
- Perform comprehensive assessments, manage symptoms, administer medications per orders, and address wound care as appropriate
- Maintain a high level of autonomy; work to the top of your license while coordinating with a multi-disciplinary care team
- Document all care activities and patient progress in the electronic medical record; update care plans accordingly
- Educate patients and families on disease management, safety, and care modalities; support caregivers to manage care at home
- Adapt to changes in patient needs and scheduling while maintaining quality and safety standards
Requirements
- Current active NH Nursing License and BLS certification
- Minimum 18 months Home Health experience
- Hospice experience preferred
- 2-5 years Med-Surg experience preferred
- Self-directed and able to work without direct supervision
- Ability to adapt to home environments and diverse patient populations
- English: speak, write, read, and comprehend
- Computer proficiency in electronic medical records data entry
- Valid driver’s license and auto insurance
- All licenses/certs current at time of submission
- Territory: South Territory – Littleton/Plymouth areas
