Responsibilities
- Conduct PT evaluations and develop/implement individualized treatment plans to restore physical function and improve mobility and daily activity performance.
- Deliver ongoing PT treatment per physician orders, monitor patient response, and adjust the plan of care as needed.
- Serve as a co-leader in an interdisciplinary approach, collaborating with other rehab professionals and care teams to guide patient groups.
- Document patient care in Cerner, ensuring accurate, timely notes and compliance with documentation standards.
- Utilize and manage therapy equipment (wheelchairs, walkers, canes) and incorporate special procedures (e.g., video swallow examinations) during scheduled times when applicable.
- Participate in weekend rotation and, where needed, support coverage across sites; coordinate with the team to optimize patient access and safety.
Requirements
- Active California Physical Therapy license
- Doctor of Physical Therapy (DPT) degree or equivalent
- At least 1 year of physical therapy experience in rehabilitation or related setting preferred
- Willingness to travel to multiple sites and work in a home healthcare/hospice environment
- Proficiency with Cerner EMR or willingness to learn the system
