The Case Management / Care Coordination role focuses on transition planning for diverse patient populations across Med-Surg, long-length-of-stay, and ED pathways, with cross-unit collaboration and block scheduling.

- Floating: Will float across units and campuses as needed to meet patient care needs

Responsibilities
- Lead discharge planning and care coordination to ensure timely, safe transitions to the next level of care for Med-Surg, long LOS, and ED populations
- Conduct needs assessments, develop and monitor plans of care, coordinate services (including DME ordering) to meet CMS requirements
- Collaborate with Social Services and interdisciplinary teams to remove barriers to discharge and optimize patient flow; document care plans in EPIC
- Monitor patient risk and ratios (approximately 25–30 patients) and prioritize care coordination activities to support safe outcomes
- Ensure compliance with hospital policies and regulatory requirements; maintain accurate, timely documentation and communication

Requirements
- Active RN license in Washington State; pending license accepted for submission
- BLS certification
- Minimum 1 year acute care experience; discharge planning / care coordination experience required
- EPIC charting experience; ability to work across Med-Surg, long LOS, and ED settings; diverse case management preferred
- Ability to work in a hospital environment and float between units and campuses as needed
