Responsibilities
- Conduct initial and ongoing PT evaluations to determine therapy needs and develop individualized plans of care for residents in a geriatric rehabilitation setting
- Implement and supervise therapeutic interventions, including therapeutic exercise, gait training, balance and mobility training, functional activities, and assistive devices as appropriate
- Reassess resident progress, document outcomes, update plans of care, and adjust interventions to maximize functional gains
- Collaborate with an interdisciplinary team (nursing, therapy assistants, case managers, and social services) to coordinate care and support discharge planning and transitions of care
- Educate residents and families on home exercise programs, safety, fall prevention, and community resources
- Ensure accurate, timely documentation in the electronic medical record and maintain compliance with regulatory and facility policies

Requirements
- Active Physical Therapy license in Maryland or eligibility to practice in Maryland
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
- Willingness to undertake a13-week travel assignment with the possibility of approved time off as noted in assignment details
- Experience in skilled nursing or geriatric rehabilitation settings preferred
- Strong communication and teamwork skills; ability to collaborate with diverse clinical staff
- Ability to document and manage care in an electronic medical record system and adhere to rehabilitation best practices
