Responsibilities
- Provide direct nursing care to residents in a memory care/long-term care setting, following the plan of care.
- Administer prescribed medications and treatments and document administration accurately in the medical record.
- Monitor residents’ vital signs, assessment changes, and overall condition; report significant findings to the supervising clinician.
- Assist residents with activities of daily living, mobility, hygiene, nutrition, and personal care while promoting dignity and independence.
- Collaborate with an interdisciplinary team to implement and update care plans; ensure safety, infection control, and quality of care.
Requirements
- Active Vermont Licensed Practical Nurse (LPN) license
- Graduation from an accredited practical nursing program
- Ability to work evening or night shifts (shifts include evening and night)
- Experience in long-term care or memory care settings preferred
- Proficiency with basic electronic documentation and communication within a care team
