Responsibilities
- Assist residents with ADLs per the plan of care: hygiene, grooming, dressing, incontinence/catheter care, toileting, turning/positioning, skin care; document per protocol.
- Maintain resident environment and safety: answer call lights promptly, use safety mats and low beds, monitor equipment, prevent falls; notify RN/Charge as needed; document per protocol.
- Support nutritional needs: assist with meal delivery, tray setup, adaptive equipment, feeding, nourishment; honor preferences; document per protocol.
- Support rehabilitative/restorative care: assist with ROM, ambulation, transfers, proper body alignment and use of assistive devices; document per protocol.
- Uphold residents' rights and report changes: report refusals, concerns, and status changes to Charge Nurse promptly; follow policies for infection control and occupational safety.
Requirements
- NYS Nursing Assistant Certification (CNA)
- BLS certification (AHA or RQI)
- One year of LTC experience (CNA) required
- Active NYS CNA license held for at least 1 year
- Education: High School Diploma or GED
- Ability to work in a skilled nursing setting and follow care plans; strong communication; ability to stand/walk for extended periods
