Responsibilities
- Provide direct nursing care to patients in a home healthcare and hospice context, with readiness to address ER-level concerns as needed.
- Assess patient condition, monitor vitals, administer medications and treatments as ordered, and respond to emergencies according to protocols.
- Perform timely triage, interpret clinical findings, and prioritize care to stabilize patients.
- Document patient status, care plans, and outcomes in the electronic health record; communicate with physicians and the interdisciplinary team.
- Educate patients and families on care plans, medications, safety, and when to seek urgent care.
- Adhere to hospice policies, infection prevention, and safety guidelines while delivering patient-centered care.
Requirements
- Active RN license in California
- ER nursing experience preferred
- Ability to work 12-hour day shifts; schedule includes 3 days per week
- Willingness to travel for a temporary assignment
- Strong communication and documentation skills; ability to work independently in a home-based setting
- Comfort functioning in a home healthcare and hospice environment
