Responsibilities
- Conduct initial physical therapy evaluations and periodic reassessments to determine patient needs and progress.
- Develop and implement individualized treatment plans aligned with short- and long-term rehabilitation goals.
- Deliver evidence-based interventions, including therapeutic exercise, gait and balance training, mobility training, and neuromuscular reeducation.
- Apply appropriate modalities and manual techniques within scope of practice and facility policies; monitor patient response and adjust treatment accordingly.
- Document assessments, treatment plans, progress notes, and discharge summaries in the facility electronic medical record; maintain clear communication with physicians and the interdisciplinary team.
- Educate residents and families on home exercise programs, safety strategies, and strategies to promote independence and fall prevention.

Requirements
- Active Physical Therapy license in Washington state or eligibility to obtain licensure promptly.
- Doctor of Physical Therapy degree (or equivalent) from an accredited program.
- Minimum 1 year of physical therapy experience, preferably in geriatric or skilled nursing settings.
- Ability to work 40 hours per week in a 5x8 shift, in a skilled nursing environment.
- Strong clinical assessment, documentation, and communication skills; ability to collaborate effectively with an interdisciplinary team.
- Willingness to travel for a 13-week assignment and start around 06/01/2026.
