In this ambulatory care RN role, you’ll be at the center of preventive health, chronic disease management, and coordinated care. Core responsibilities include patient triage and assessment, timely documentation in electronic health records, immunizations and preventive services, wound care and basic procedures, medication reconciliation, and patient education on disease management, self-care strategies, and adherence to treatment plans.
