- Background checks and clearances must be completed within defined timeframes (7-year nationwide background, county searches, FACIS3, sex offender registry; FACIS 3 and VCIC within 30 days)
Responsibilities
- Provide direct patient care to adults in a long-term care setting, including routine nursing tasks and medications per orders, with documentation in the medical record.
- Conduct ongoing patient assessment, monitor vital signs, and promptly escalate changes to the supervising RN/MD.
- Implement and update care plans; assist residents with activities of daily living; support dementia care and safety measures to prevent falls.
- Collaborate with interdisciplinary team members, communicate with supervisors, and ensure accurate handoffs during shift change.
- Maintain infection control practices, ensure resident comfort, and be prepared to float to other units as needed; every other weekend availability is required.
Requirements
- Active LPN license in Vermont (or eligible and able to obtain licensure in Vermont)
- BLS certification through the American Heart Association
- Minimum 3 years of LPN experience, with LTC and dementia care experience preferred
- Prior travel experience is preferred
- Ability to work Monday–Sunday schedule with no block scheduling; willingness to float to other units as needed
- Must be able to pass the required background checks: 7-year nationwide background, county searches, FACIS3, and sex offender checks within defined timelines; FACIS 3 and VCIC verification within 30 days of start
- Comfortable working with residents who have dementia; reliable transportation or access to transportation is recommended due to limited public transit in the area
