Responsibilities
- Provide routine nursing care to residents under the supervision of an RN, following care plans and facility policy
- Monitor residents’ conditions, document vital signs, and report changes promptly
- Administer prescribed medications and treatments per physician orders and state regulations
- Assist residents with activities of daily living and maintain a safe, clean, and compassionate care environment
- Collaborate with interdisciplinary team to implement care plans and participate in rounds and care conferences
- Maintain accurate recordkeeping and documentation in the electronic health record, ensuring confidentiality and compliance
Requirements
- Active Licensed Practical Nurse license (state to be verified during onboarding)
- Ability to work a 3x12-hour shift schedule (long-term care setting)
- Experience in or demonstrated ability to work in a long-term care or skilled nursing environment
- Strong communication and teamwork skills; comfortable coordinating with nursing staff and other caregivers
- Ability to follow care plans, maintain resident safety, and document care accurately
