Responsibilities
- Provide direct patient care within the licensed practical nurse scope, including medication administration under supervision, vital signs, and routine treatments
- Monitor residents for changes in condition; document findings and report significant changes to the supervising RN
- Perform wound care, dressing changes, and activities of daily living assistance to residents as appropriate
- Apply care plans in collaboration with the interdisciplinary team; ensure care is consistent with physician orders and facility protocols
- Maintain patient safety and infection prevention practices; complete accurate and timely charting in the electronic health record
- Assist with admissions, transfers, and discharges as needed; participate in shift handoffs and care coordination
Requirements
- Active LPN license in good standing in New Hampshire
- CPR/BLS certification current or eligible for recertification
- Experience in long-term care or skilled nursing settings preferred
- Ability to work Evening shifts (3:00 pm–11:00 pm) and participate in a travel assignment
- Strong communication skills, teamwork, and attention to detail
- Ability to work in a fast-paced, team-based environment and follow facility policies
