Registered Nurse, Case Manager | Grand Junction, Colorado | Travel – Temporary Assignment. Start date 05/04/2026 for 13 weeks. Weekly pay range: $1,874–$1,891 per week. Guaranteed hours: 40 per week. Responsibilities
- Manage a caseload of patients as the primary RN in case management, coordinating care with the interdisciplinary team.
- Conduct comprehensive assessments, identify patient goals, and develop/modify care plans to align with goals of care.
- Educate patients and caregivers about disease processes, care options, symptom management, and available community resources.
- Coordinate referrals, services, and transitions of care across home, home health, nursing homes, and other community settings.
- Document care plans, interventions, and patient progress; communicate changes promptly with physicians and team members.
- Lead and participate in the interdisciplinary team to ensure timely delivery of quality, patient-centered care.
Requirements
- Active registered nurse (RN) license in Colorado
- Experience in case management or care coordination; hospice/palliative care experience preferred
- Strong assessment, critical thinking, and clinical decision-making skills
- Excellent communication and collaboration with interdisciplinary teams
- Ability to work effectively in outpatient and home/community setting environments
