Responsibilities
- Administer skilled nursing care in the patient’s home and coordinate care with the interdisciplinary team, patient/family, and referring agencies.
- Assess home health and hospice patients in stable and emergent situations; identify physical, psychosocial, and environmental needs and document findings; develop and evaluate the Plan of Care (POC).
- Maintain up-to-date patient records, ensuring visits, contacts, problems, plans, actions and goals are accurately recorded and updated as changes occur.
- Educate patients and families regarding care, medications, home safety, diet and other self-management topics; tailor teaching to age, condition and cultural context to promote self-care.
- Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure quality and continuity of service.
- Communicate effectively with patients, families, team members, and other health professionals; participate in interdisciplinary team meetings and coordinate referrals or on-call duties as needed.
Requirements
- Current Washington State RN license; CPR certification; valid driver’s license with reliable transportation
- 1-2 years of RN clinical care experience; 2+ years of home health experience
- OASIS experience and Homecare Homebase software experience required
- Education: ADN or Nursing Diploma required; BSN preferred
- CHPN certification preferred
