Responsibilities
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and care team
- Contribute to development of an age-appropriate care plan with nursing diagnoses and interventions; involve resident/family and interdisciplinary team
- Recommend revisions to the plan of care based on resident outcomes and coordinate referrals to address all aspects of care
- Provide direct and indirect skilled nursing care under defined policies and under RN supervision; maintain patient safety and environment
- Maintain current nursing practice knowledge; participate in continuing education and Inservice programs; mentor or orient less-experienced staff
- Demonstrate leadership in the professional practice setting and collaborate with physicians, other nursing staff, and ancillary team members to support geriatric residents
Requirements
- Active Licensed Practical Nurse license in New York
- BLS certification through American Heart Association (AHA) or Resuscitation quality improvement (RQI)
- Minimum 1 year of LPN experience in a long-term care facility
- LPN diploma or equivalent education
- Ability to provide care to adult and geriatric residents; ability to follow under RN supervision; ability to work in a skilled nursing environment
