Responsibilities
- Provide direct patient care under the supervision of a registered nurse, including monitoring health status, administering prescribed medications, and delivering routine treatments.
- Assess and document patient conditions, vital signs, and changes in status; report clinically significant findings to the supervising RN promptly.
- Assist residents with activities of daily living (ADLs) and basic comfort measures while promoting dignity and independence.
- Maintain accurate and timely documentation in the patient charting system and ensure patient safety through proper device management and infection control.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans; respond to emergencies and ensure smooth patient transitions.
Requirements
- Active, unrestricted LPN license to practice in New Mexico.
- Experience in long-term care or geriatric patient care preferred.
- Ability to perform 12-hour shifts and meet guaranteed hours per week.
- Strong communication skills and ability to work collaboratively with a care team.
- Adaptability to a fast-paced, resident-centered LTC environment and commitment to resident safety and quality of care.
