This is a travel - temporary assignment in a long-term care environment, providing care to adult and geriatric residents under RN supervision.

Responsibilities
- Collect data on residents' physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team
- Contribute to development of an age-appropriate, culturally relevant plan of resident care; involve resident/family; promote a healthy, safe environment
- Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to address all aspects of care; collaborate with the resident/family and Interdisciplinary Team
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of an RN; maintain current nursing knowledge and participate in ongoing education and in-service programs
- Mentor and orient staff as needed; assume leadership responsibilities in the professional practice setting; uphold safety and regulatory standards; maintain awareness of exposure to bloodborne pathogens
Requirements
- Active NY Licensed Practical Nurse license
- BLS certification (AHA or RQI)
- At least 1 year of LPN experience; LTC facility experience preferred
- Ability to work in a long-term care environment and participate in D/E/N/W shift rotations
- Must be able to start full-shift orientation on a Tuesday or Thursday
- Holiday coverage may be required
