Responsibilities
- Conduct initial assessment and triage for assigned patients, with attention to urgent symptoms in a home-based hospice context
- Administer medications and treatments per plan of care and physician orders; monitor for safety and effectiveness
- Track vital signs, respond to changes, and escalate to the supervising clinician as needed
- Document care clearly and accurately in the electronic health record; clock in to the unit when covering a specific unit per float pool policy
- Support symptom management, comfort measures, and end-of-life care planning; provide education to patients and families
- Collaborate with the interdisciplinary team to coordinate care across shifts and settings
Requirements
- Active Registered Nurse license in California
- Ability to work 12-hour night shifts
- Willingness to participate in a travel assignment and cover float pool needs
- Proficiency with electronic health records and accurate documentation
- Strong communication, critical thinking, and teamwork skills
- Ability to function in a home healthcare – hospice environment and adhere to care plans
