Responsibilities
- Conduct initial physical therapy evaluation and establish a Plan of Care for each resident
- Implement and monitor individualized therapy programs addressing mobility, strength, balance, gait, and activities of daily living
- Reassess residents regularly, document progress, and update treatment plans as needed
- Collaborate with the interdisciplinary team to coordinate care, discharge planning, and goals
- Ensure patient safety and maintain compliance with regulatory standards and documentation requirements
- Communicate clearly with residents, families, and team members regarding goals, progress, and changes in therapy
Requirements
- Active Physical Therapist license in good standing
- CPR/BLS certification
- Ability to work in a skilled nursing care setting and with geriatric populations
- Availability for a 13-week travel assignment with guaranteed 35 hours per week
- Strong documentation and communication skills; reliability and teamwork
