Responsibilities
- Provide inpatient case management, discharge planning, and care coordination for diverse patient populations (neonates through geriatrics)
- Conduct needs assessments, develop and monitor plan of care, arrange DME and post-discharge services
- Collaborate with interdisciplinary teams, including Social Services, to optimize patient outcomes and ensure timely transitions
- Document and communicate plan of care in the charting system (EPIC) and ensure CMS/compliance requirements are met
- Manage a caseload with a focus on safe, timely discharges and appropriate referrals to post-acute services
- Support care transitions across Med-Surg, ED, and ancillary services as needed

Requirements
- Licensure: Active WA RN license or eligibility for WA Compact RN; or active MSW licensure as applicable; BLS required
- Certifications: BLS; EPIC charting experience (required)
- Experience: Minimum 1 year of acute care experience with discharge planning/care coordination; behavioral health experience does not count as acute care hospital experience
- Education: RN or MSW licensure as applicable
- Practical: Ability to work in an inpatient hospital setting; willingness to float across hospital areas and two campuses; pending license accepted (submission status required)
- References: 2 references covering 1 year within the last 3 years
