Travel Registered Nurse Case Manager Job
Registered Nurse (Case Manager) – Care Coordination / Discharge Planning in Everett, WA. This travel assignment starts 05/18/2026 for 13 weeks. Earn $1,692–$1,815 per week. Shift: 4x8 days. Guaranteed hours: 72 hours per 2-week pay period. Setting: hospital, Level II trauma center, with care coordination and discharge planning across acute care units for patients from Neonates to Geriatrics.

Responsibilities
- Provide case management, discharge planning, and care coordination for acute care inpatients (neonates through geriatrics)
- Conduct needs assessments, coordinate DME ordering, and develop/adjust the patient plan of care
- Collaborate with Social Services and interdisciplinary teams to support safe transitions of care
- Document and communicate in the EPIC charting system; lead patient/family education and care transitions
- Manage typical patient load within Case Management (approx. 25–30 patients) and participate in intercampus coverage
- Participate in weekend and holiday coverage as required; support pre- and post-discharge processes
Requirements
- Active RN licensure in Washington or ability to obtain; pending license accepted
- BLS certification
- Minimum 1 year of acute care experience; required discharge planning / care coordination experience
- Strong preference for diverse case management background (Med-Surg, LLOS, ED)
- EPIC charting experience required
- Ability to work across two campus locations (within a short distance) and float as needed
- Weekend every-other-weekend commitment; holiday coverage as required
- First-time travelers accepted
