Travel Physical Therapy Assistant LTAC Job
Physical Therapy Assistant (LTAC) in Hilo, Hawaii. Start date 09/14/2026 for a 13-week travel assignment in a long-term care/nursing home setting. Weekly pay range $1,371–$1,475 per week. No guaranteed hours. Shift is rotating to cover department needs with day-shift emphasis and potential weekend coverage. This is a travel – temporary assignment through Healthforce Healthcare LLC.

Responsibilities
- Provide rehabilitation services in a skilled nursing facility and/or LTAC setting, including evaluation, reassessment, and treatment planning within licensure and scope of practice
- Develop, implement, or carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team
- Apply evidence-based therapeutic interventions to maximize function, mobility, communication, swallowing, independence, and quality of life
- Educate residents, families, and caregivers about treatment plans, progress, and discharge goals
- Complete accurate, timely, and compliant documentation; monitor progress and adjust plans of care as needed
- Supervise therapy assistants when applicable and coordinate care within the rehabilitation department
- Be available to work Hawaii State and Federal Holidays as necessary; may float to other units or facilities based on census and staffing needs
- Complete onboarding including department orientation and EMR training to ensure seamless integration into daily workflows
Requirements
- Current Hawaii license in good standing appropriate to the Physical Therapy Assistant scope of practice; Hawaii license must be active by start of assignment or applicant must apply upon acceptance
- Current Basic Life Support (BLS) certification
- Minimum two (2) years of experience in the SNF/ICF setting preferred
- Ability to work in a LTAC/SNF environment and comply with state and federal regulations
- Orientation and training familiarity with EMR systems as part of onboarding
