Responsibilities
- Plan and conduct segments of treatment programs to restore and improve physical function, participation, and quality of life.
- Perform initial evaluations, ongoing re-assessments, and establish individualized treatment plans using evidence-based protocols.
- Provide therapy interventions and educational guidance per approved treatment protocols; adjust plans as needed.
- Serve as co-leader in an interdisciplinary team approach for patient groups; coordinate with other disciplines to optimize outcomes.
- Develop individualized care and discharge plans; communicate plans to patients, families, and care teams.
- Complete comprehensive and accurate clinical documentation within established deadlines; supervise assigned assistants.

Requirements
- Doctor of Physical Therapy (DPT) degree is required; and per notes, this applies when a license was obtained after January 2016, or for foreign equivalents.
- Physical Therapy licensure in the state of practice is required.
- Cardiopulmonary resuscitation (CPR) or Basic Life Support (BLS) certification from an approved vendor per policy is required.
- Two years of Physical Therapy experience is preferred.
- Ability to work in an outpatient caseload and support a 4x10-hour shift schedule; ability to collaborate effectively within an interdisciplinary team.
