Responsibilities
- Perform comprehensive physical therapy evaluations for residents to assess mobility, strength, ROM, balance, endurance, and functional needs
- Develop and implement individualized treatment plans to improve mobility, gait, transfers, strength, and activities of daily living
- Administer therapeutic interventions (therapeutic exercise, gait training, balance activities, manual therapy, modalities as appropriate) and progress as tolerated
- Document evaluation, treatment progress, goals, and outcomes in the designated rehabilitation software (NetHealth Rehab Optima) and coordinate with the interdisciplinary team
- Reassess residents regularly, modify plans of care, and communicate progress to physicians, nursing staff, families, and other care team members
- Ensure safety, infection control, and compliance with facility policies and regulatory standards
Requirements
- Active California Physical Therapy license
- Minimum 1 year SNF experience preferred
- NetHealth Rehab Optima knowledge preferred
- Ability to work the Day shift and commit to 38 hours per week
- Willingness to participate in a 13-week travel assignment in Monterey, CA
