Responsibilities
- Conduct biopsychosocial assessments and develop comprehensive care plans for adult and geriatric inpatients.
- Lead discharge planning and coordinate post-acute services with internal teams and community resources.
- Collaborate with an interdisciplinary team (PT, Interpretation Services, Social Services, Pharmacy, Transportation) to ensure timely, safe transitions of care.
- Manage complex cases, including guardianship issues and high-risk interventions; navigate Medicare/Medicaid processes as needed.
- Document thoroughly in EPIC and maintain accurate, confidential patient records; adhere to policy and regulatory standards.
- Float across hospital areas as needed, participate in 40-hour orientation, and respond to variable caseloads (typical 18–24 patients; weekends may exceed 30).

Requirements
- Active Washington clinical license in-hand (LICSW or approved clinical equivalents listed: LSWAIC associate, LASW, LSWAA associate) and BLS.
- Education: Master’s degree in Social Work; facility accepts LMSW or LCSW (Master’s degree required).
- Experience: Minimum 1 year of acute care experience with discharge planning/care coordination; experience with diverse case management across Med‑Surg, long LOS, and ED preferred.
- Practice setting: Ability to work in an adult/geriatric inpatient population and perform psychosocial interventions.
- Scheduling and logistics: Willingness to float across multiple hospital areas; orientation and pre-start requirements completed prior to start.
- References and background: Minimum 2 supervisory references covering 1 year within the last 3 years; locational restrictions apply (locals not accepted).
