Responsibilities
- Provide direct nursing care to residents in a long-term care setting under facility policy and physician orders
- Administer prescribed medications and document accurately in the patient record
- Monitor residents' health status, assess vital signs, identify changes, and report to the supervising nurse
- Participate in care planning, collaborate with the interdisciplinary team, and maintain clear, timely documentation
- Support safe resident care through infection control practices, safety protocols, and quality standards
- Communicate effectively with residents, families, and the care team to ensure continuity of care
Requirements
- Active Licensed Practical Nurse (LPN) license; valid for practice in the United States
- Basic Life Support (BLS) certification
- Experience in long-term care or skilled nursing settings preferred
- Ability to work the evening shift (3:00 pm–11:30 pm) and meet guaranteed-hour expectations
- Strong clinical documentation and communication skills
- Education: graduate of an approved practical nursing program or equivalent credential
