Responsibilities
- Implement and monitor a physical therapy treatment plan under supervision, focusing on mobility, strength, and functional activities for LTAC/SNF patients.
- Assist with patient evaluations, progress documentation, and data collection as directed by the supervising PT.
- Instruct patients and families on prescribed exercises, transfers, gait training, posture, and activities of daily living; promote safety and independence.
- Assist with therapeutic modalities, ROM exercises, gait aids, transfers, and assistive devices as directed; ensure proper positioning and safety during therapy.
- Prepare treatment areas and equipment, maintain clean and safe environments, and follow infection control procedures.
- Collaborate with the interdisciplinary team to support discharge planning and optimize patient outcomes.
Requirements
- Active New York license as a Physical Therapy Assistant
- BLS certification
- Prior experience in a Skilled Nursing Facility (SNF) REQUIRED
- Ability to work in a skilled nursing facility setting
- Availability to work Tuesday through Saturday, 7:30 a.m. to 4:00 p.m. (8-hour shifts)
