Responsibilities
- Provide physical therapy evaluation and treatment for residents in a skilled nursing setting
- Develop and implement individualized treatment plans to address functional limitations and mobility
- Perform therapeutic modalities, therapeutic exercises, gait training, and balance activities as indicated
- Reassess progress regularly, document outcomes, and adjust plans accordingly
- Collaborate with an interdisciplinary team to support discharge planning and ongoing rehabilitation goals
- Maintain accurate clinical documentation and ensure compliance with regulatory and facility protocols
Requirements
- Active physical therapy license (PA license or eligibility for PA) and ability to practice in the United States
- Doctor of Physical Therapy (DPT) or equivalent professional degree
- Relevant experience in a skilled nursing facility or similar setting preferred
- Strong documentation and communication skills; ability to work within a multidisciplinary team
- Ability to work in a long-term care environment and adapt to patient needs
