Responsibilities
- Deliver direct patient care to residents in a long-term care setting, including medication administration, routine assessments, and wound care as directed.
- Monitor vital signs, observe changes in condition, and report findings to the charge nurse; document accurately in the charting system.
- Collaborate with the interdisciplinary care team to implement and update individualized care plans; assist residents with activities of daily living as needed.
- Maintain infection control practices, patient safety protocols, and adherence to approved policies and procedures; respond promptly to call lights and emergencies.
- Complete required nursing documentation in a timely manner and ensure accuracy in the electronic health record.
- RTO: 11/30

Requirements
- Active Licensed Practical Nurse license in New Mexico.
- Ability to work the night shift (17:00–05:30) on a 3x12 schedule.
- Experience in a long-term care or skilled nursing setting is preferred.
- Strong communication and documentation skills; ability to work effectively within a care team.
- Must meet standard health and safety requirements and complete onboarding as required.
