Responsibilities
- Provide direct patient care under supervision of licensed staff, including assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming
- Support residents with mobility and transfers using proper body mechanics and assistive devices
- Monitor and report changes in resident condition, safety concerns, and vital signs as directed by the supervising nurse
- Assist with meals, including feeding as needed, and document intake and output according to care plans
- Maintain cleanliness and safety in resident rooms and common areas; adhere to infection control and safety protocols
- Follow individualized care plans and record care activities accurately and promptly
Requirements
- Active Certified Nursing Assistant (CNA) license in New Hampshire
- Ability to work a 12-hour shift schedule and the assignment’s rotating days
- Strong communication and teamwork skills; ability to follow care plans
- Experience in a long-term care setting is preferred but not required
- Must meet onboarding credentialing and background check requirements as part of the assignment
