Responsibilities
- Provide direct and indirect skilled nursing care to residents, under supervision of an RN.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Assist in developing an age-appropriate, culturally relevant plan of care; involve residents and families in care planning.
- Recommend revisions to the plan of care based on resident outcomes and coordinate referrals as needed.
- Collaborate with residents, families, and the Interdisciplinary Team to ensure comprehensive care and a safe environment.
- Maintain current nursing knowledge and competencies; participate in orientation/mentoring of staff; demonstrate leadership within the clinical team.
Requirements
- Current New York LPN licensure
- BLS certification (AHA or RQI)
- Minimum 1 year of experience as an LPN in a long-term care setting
- Ability to provide care for adult and geriatric residents
- Capacity to work under the direction of an RN and in collaboration with the care team
- Availability for a 40-hour per week schedule, including rotating days/evenings and weekend/holiday coverage
