Responsibilities
- Conduct comprehensive evaluations of speech, language, cognitive-communication, and swallowing in adult residents; establish diagnoses and therapy goals.
- Develop, implement, and monitor individualized treatment plans; provide direct therapy and modify plans based on progress and interdisciplinary input.
- Collaborate with nursing, rehabilitation, social work, and medical teams; participate in care planning meetings and family conferences as appropriate.
- Document evaluation results, treatment notes, progress, and discharge plans in the electronic medical record; ensure accuracy, privacy, and compliance with regulations.
- Educate residents, families, and caregivers about communication strategies and swallowing safety; assist with discharge planning and community referrals.
Requirements
- Active Speech-Language Pathology license in New Hampshire or eligibility for licensure
- Ability to work in a skilled nursing facility environment and participate in an interdisciplinary team
- Strong assessment, diagnostic, and treatment planning skills for adult populations
- Proficiency in documentation and clear communication with patients, families, and staff
- Availability to work the 08:00–16:30 shift with a guaranteed 35 hours per week on a 13-week contract
